Be able to ask a question or 2 ( custom fields) prior to member signing Waiver

We would like to be able to ask a couple of required  questions during the waiver signing process -- the signature would only need to apply to the waiver as long as the answers are recorded ( perhaps in their profile ) or somewhere else that can be reported on -- Sort of like a mandatory survey that is required  at first login.
