Add custom field for registrants to league set up

We are running leagues with Swish outside of the court reserve app since it is not fully integrated.  In order to reduce confusion for league registration, it would be very helpful to have the custom fields to know teammates and other information.

We have team registration being released on July 8th fully integrated into Swish which will make this much more seamless!
