Optional (Yes/No) Waivers

The option for a one-time waiver to be displayed when registering for a membership OR signing up for a lesson/program/event where the customer can select YES or NO. For example, the liability waiver is mandatory for all participants of the Club, but a MEDIA WAIVER should be optional. For example, I may agree to join the club and sign the media waiver, but I don't wish for my photo to be taken by the club and used on our social media (or CR's member portal).
