Assign Waivers requirements based on Lessons or Courts Bookings

We need to be able to assign different waivers when Members are booking a specific Type of Court or specific Lessons Type. This will facilitate to expand the usage of our platform to other areas like Golf and Yoga, etc. As they require different kind of waiver details.
