Wavier Check Box

We have a QR code sanner and a waiver online but if they fill out the waiver on the QR code that sends them to our courtreserve waiver singing it does not link to there account and then the little yellow box is always right next to there name because they know they alresdly did it so they don't need to do it again. So I think we should add something that says "hey this person alreadly filled it out". So however is at our front desk knows that person filled out the waver before and the yellow box is gone.
