Add Notes field on Rating tab

Before the Ratings tab was added, there was a notes field just below the ratings detail fields.  It was helpful to add detail around a member's rating.  While that notes field is still present on the Details tab, it would be helpful to have a notes field on the Ratings tab.  We often enter notes around a member, and having the notes field on the Detail tab isn't appropriate for Ratings detail.
