Simple sign up form for guests with basic information required
We need to have 2 sign up forms based on Membership. We need one comprehensive one for Members and one simple sign up form that if its a guest we just need name, email and phone.

Love this idea!
Would work for us as well for new players who want to learn to play (one time), then no longer are interested in continuing. Our membership form currently requires all club members to be active national members (for a fee) prior to joining as a member to the club. A second form would allow a person to check the sport out (without the fee) prior to joining the national member assoc and then club.
