Custom heading and field labels for each Guest Waiver Kiosk

Every Guest Waiver Kiosk shows the same heading ("Guest Form & Waiver For [Organization]") and the same First Name, Last Name, Email and Phone labels. We run two kiosks from QR codes on our door: one for adults, and one where a parent signs for their children. On a phone they look identical, and on the child form a parent can easily type their child's name into the name fields.

Please let us set the heading for each kiosk (or show the kiosk's Name, which already exists but never appears on the form), and relabel the built-in fields per kiosk, for example "Parent or guardian first name".
