Guest tracking through kiosks

Could there be a way to add a “guest” function in the kiosks mode? Once the guest function is activated it would take you to a form where we can put our liability waiver and a place to input the guests name, member name, email/phone, and if they want to be contacted. Once they have filled out the waiver we can connect it to a member on our end.
